Keepsakes in Hades are special items that can be equipped on Zagreus that grant him different types of benefits and passive bonuses during a run. Players will not have access to Keepsakes at the start of the game, and they will become available later on. To acquire some, players have to give Nectar to the different NPCs they encounter to receive a Keepsake from them in exchange. These Keepsakes function like accessories would in other games, and players can only equip exactly one Keepsake at a time. Nectar can be found as reward for clearing rooms that have its icon on the door or can be purchased from the Wretched Broker, but in the latter's case, only one per run can be acquired.

What are Keepsakes in Hades
Keepsakes function like accessories in Hades. Zagreus is able to equip one Keepsake at a time, and each one grants specific passive bonuses. These Keepsakes, as the name suggests, are special items that you can collect from certain NPCs as a way of remembering them, in terms of in-game lore.
Once acquired, players can choose which Keepsake to equip using the Keepsake cabinet that can be found in Zagreus' room in the House of Hades. The Keepsakes cabinet can also be found in the courtyard where you can test out Infernal Arms and switch them up before a run. As mentioned, Zagreus can only equip one Keepsake at a time, and one Legendary Keepsake or Companion in addition. There is an upgrade you can build through the House Contracter that can allow you to switch Keepsakes mid-run, with limitations, and this doesn't apply to Legendary Keepsakes.
How to Acquire Keepsakes in Hades
Keepsakes are primarily acquired whenever you give a Nectar to a specific character for the very first time. These NPCs will each give a different Keepsake which will give effects that are in line with their different personalities and natures. For Gods and Goddesses, you'll have the option to give them Nectar whenever you come across them to select a Boon during your run. Hades and Persephone are exceptions to this. Both will require you to clear the game a few times, while Hades will require you to be on the lookout for when the option to give him gifts is actually available in between your runs. You'll have to give him Nectar twice to get his Keepsake.
Legendary Keepsakes in Hades
Legendary Keepsakes or Companions are special Keepsakes that allow you to summon the aid of a friend for a period of time. They can only be summoned once per escape attempt at first. They are acquired by completing certain requirements with regard to certain characters. After following their individual storylines and seeing their stories through, you'll have the opportunity to give them Ambrosia. Giving each of them Ambrosia will have them reward you with the Legendary Keepsake tied to them.

Hades Keepsakes Guide
Can you Switch Keepsakes in Hades?
Initially, players will only be able to switch Keepsakes in between runs, through the cabinet in Zagreus' room in the House of Hades. As such, while being able to manipulate your chances of getting the Boon from the God or Goddess you want and have it be a rare one is valuable, Keepsakes that do this only work once, so it can be disadvantageous to choose them over others.
However, eventually, players will be able to put in a work order with the House Contractor to renovate the Keepsake cabinet into the Keepsake Collection, Regional. This will allow you to access the your new and improved Keepsake cabinet in between Locations in the Underworld. This means that when you reach the end of an environment, you can switch up your Keepsakes before you enter the next one. However, you cannot switch your Legendary Keepsake or Companion through this method.
How to Upgrade Keepsakes in Hades
Keepsakes are upgraded automatically by clearing rooms in the Underworld. Each Keepsake can be upgraded for a total of three times, indicated by the stars under their images in the Keepsake cabinet, which increases their bonuses. The Keepsake must be equipped in order for the cleared rooms to count toward its upgrade. To reach the 2nd Rank, a Keepsake must be equipped while the player clears 25 rooms of Enemies in the Underworld. Once reached, the player will then need to clear 50 more rooms to reach the max rank of 3.
Hades How to Upgrade Legendary Keepsakes
Legendary Keepsakes or Companions can also be upgraded. As mentioned previously, they cannot be switched in between Locations in the Underworld, unlike the normal Keepsakes, but they can still be accessed with the improved Keepsake cabinet and then upgraded through there. Legendary Keepsakes can be upgraded up to four times for a maximum rank of 5 in exchange for certain amounts of Ambrosia. Players will need 1 for the 2nd Rank, 2 for the 3rd Rank, 3 for the 4th Rank, and 4 for the final Rank, for a total of 10 Ambrosia to fully upgrade one Legendary Keepsake. Each upgrade allows you to summon a Companion one additional time per run.
